卡铂按AUC给药的计算方法：

卡铂（Carboplatin）的剂量限制毒性主要是骨髓抑制，其血小板减少较粒细胞减少更为严重。卡铂无明显肾毒性，但治疗前肾功能状况却能显著影响卡铂所致血小板减少的程度。动物实验表明，卡铂的肾脏清除完全由肾小球滤过而完成，无肾小管的参与，胆汁排泄低于2%。早期研究注意到，治疗前肾小球滤过率（glomerular filtration rate，GFR）低下者血小板减少较普遍，因此研究者曾推荐按照GFR调整卡铂剂量。1984年Egorin等提出，静脉给卡铂后，血小板最低值与游离卡铂AUC（曲线下面积）相关。Calvert等也认为，主要根据GFR在60～100ml/min时确立的单药卡铂剂量400mg/m2，对于高GFR者并不适合。因此，根据GFR的变量改变影响游离卡铂AUC与所给剂量的线性关系，提出了卡铂按AUC给药的计算方法。这样计算出的卡铂剂量，可取得最大疗效并避免不可耐受的毒性。

卡铂按AUC给药的计算方法:

主要是根据Calvert公式进行。首先确定以下条件：性别、年龄、体重、血清肌酐和AUC，代入公式，即可算出卡铂应使用的剂量。

1、Calvert公式：卡铂剂量（mg）=所设定的AUC（mg/ml/min）×［肌酐清除率（ml/min）+25］

2、由于肌酐清除率检测复杂，不常进行此项检查，但可以通过血清肌酐（Serum Creatinine）来计算肌酐清除率。请注意：男性与女性肌酐清除率的计算方法有所不同。

男性肌酐清除率（ml/min）＝｛［140－年龄（岁）］×体重（Kg）×1.23｝÷血清肌酐（μmol/L）

女性肌酐清除率（ml/min）＝男性肌酐清除率（ml/min）×0.85

3、AUC（mg/ml/min）取值：常取5～7，我们常取5。

以下是摘自martindale第35版关于卡铂给药的描述：Administration.Pharmacokinetic studies by Calvert and colleagues1 have indicated that the dose of carboplatin to produce a desired area under the concentration-time curve (AUC) could be calculated, based on the patient's glomerular filtration rate (GFR), as:Dose in mg = target AUC X (GFR + 25)

It should be noted that the resultant dose is given in mg and not in mg/m2. This formula was found to be useful in patients with higher than normal as well as reduced GFR. Suggested target AUCs were 5 mg/mL per minute in previously treated patients and 7 mg/mL per minute in those who had not previously received chemotherapy. In combination therapy the appropriate AUC value depended on the other drugs used: an AUC of 4.5 mg/mL per minute gave acceptable results when carboplatin was used with bleomycin and etoposide for testicular teratoma.
