骨性关节炎放射X线诊断标准Kellgren ＆Lawrence（1957年）分级法：

在流行病学的研究中，大多数学者仍然使用Kellgren ＆Lawrence的放射诊断标准。该标准将骨关节炎分为五级：

0级 正常（X线无异常）；

Ⅰ级 可能有骨赘（疑似骨赘形成）（轻微骨赘），关节间隙可疑变窄；

Ⅱ级 有明显的骨赘，（未累及关节间隙）关节间隙轻度变窄；

Ⅲ级 中等量骨赘，关节间隙变窄较明显（关节间隙中度狭窄）（关节间隙明显狭窄），软骨下骨质轻度硬化改变，范围较小；

Ⅳ级 大量骨赘形成，可波及软骨面，关节间隙明显变窄（甚至消失），（软骨下骨）硬化改变极为明显，关节肥大及明显畸形。

这一标准属于放射学评价，而且是对骨赘的作用强调的较多，此标准仍存在较大的争议。本人认为这一标准太笼统，没有量化，临床实践中难于把握，每个临床医生之间的结果没有可比性，不便于交流和指导科研和临床工作。

（a） Grade 0: Physiological joint.

（b）Grade I: Subtle JSN in the medial compartment with osteophytic lipping.

（c） Grade II: Definite JSN in the medial compartment.

（d） Grade III: Definite JSN in the medial compartment and sclerosis of the subchondral bone.

（e）Grade IV: JSN with a bone-on-bone phenomenon and deformity of the edial tibial plateau as well as the medial femoral condyle.
